nH-Engage app for iPhone and iPad

nH Engage helps hospitals, payors, ACOs, and post-acute care providers identify patients who are considering going to the emergency room before they do so, uncover patients who are unknowingly failing at home, and alert clinicians or navigators of at-risk patients so they can engage the patient before a costly negative event occurs such as unnecessary emergency department visit or readmission.
Patients are registered by a clinician using the nH Engage system. Clinicians create checklists of questions which the patient answers every day either via this mobile application or automated phone calls. Clinicians then track their patients’ answers and respond accordingly either via phone call, video call or text message. nH Engage can be deployed for patients receiving post-acute care services or patients discharged to self-care to monitor and track their progress and actions.
